Community-Based Supports
Golden Heart’s Community Based Supports provide individualized assistance to help people safely and independently participate in everyday community life. Supports are tailored to each person’s needs, preferences, and goals and may include errands, appointments, recreational activities, accessing community resources, and other daily activities that promote choice, independence, and meaningful participation.
Program Eligibility: Community Based Supports are available to individuals enrolled in the Community Care Program (CCP).

Individual Supports
Golden Heart’s Individual Supports provide personalized, one-on-one assistance to help individuals build independence, maintain daily routines, and safely participate in home and community life. Supports are tailored to each person’s needs, preferences, and goals and may include assistance with daily living skills, personal care, household tasks, appointments, decision-making, and other activities that support greater independence and quality of life.
Program Eligibility: Individual Supports are available to individuals enrolled in the Supports Program (SP).
